Initial Visit
All services provided by Dr. Medina will begin with a 1-hour initial intake session. You will be asked to complete a packet of client forms before or at your first appointment. At the end of your first appointment, Dr. Medina will recommend a service plan for your child.
Fees
Dr. Medina will provide you with an
estimate of the cost of the recommended services during your initial visit. These services will be billed at the hourly rate. Other billable services may include a school observation, attendance at meetings with other professionals you have authorized, telephone conversations of over 15 minutes, and the time spent performing any other service you may request.
Fees for the initial intake session are collected at the time of the appointment. If an evaluation is recommended for your child and you elect to move forward with services, payment for 50% of the balance is due at the first testing appointment. Payment for the remaining balance is due at the follow-up parent meeting, after testing is completed. Payment may be made by cash, check, or credit card (Visa or Mastercard) at the time of service. For each appointment that is scheduled, 24 hours advance notice of cancellation is required in order to avoid a late cancellation fee.
Insurance Reimbursement
Dr. Medina does not accept medical or mental health insurance at this time and will not bill your services directly to your insurance carrier. If you have a health insurance policy, however, it will usually provide some coverage for mental health treatment. Although she will not bill your carrier directly, she will fill out forms and provide you with whatever assistance she can in helping you receive reimbursement for the services you have paid for. You (not your insurance company) are responsible for full payment of her fees. It is very important that you find out exactly which mental health services your insurance policy covers if you wish to submit a claim for reimbursement. Authorization is often required in advance. You are advised to call your insurance company directly to find out their policies with regard to psychological services.
Confidentiality
In general, the privacy of all communications between a patient and a psychologist is protected by law, and Dr. Medina can only release information about your child's work with her to others with your written permission. Your child's mental health information will be kept confidential unless Dr. Medina believes you or your child may harm him/herself or someone else; if it becomes known that a child, elderly adult, or disabled person is being mistreated; or if a judge orders it.
You are also protected under the provisions of the Federal Health Insurance Portability and Accountability Act (HIPAA). This law insures the confidentiality of all electronic transmission of information about you. Whenever Dr. Medina transmits information about you electronically (for example, sending bills or faxing information), it will be done with special safeguards to insure confidentiality.
